New practical guidance has been published to assist local authorities and health communities in making a strategic shift to prevention and early intervention. It draws on the experiences and evidence emerging from the first two years of the Partnerships for Older People Projects (POPP) programme and other related initiatives. The POPP programme comprises 29 pilot sites. The first round of 19 sites started in May 2006 and the second round of a further 10 sites commenced in May 2007. The sites are very varried in their approaches. Brief summaries of what they are delivering can be found by following the links below.

The POPP programme is seen as a very important test bed for the preventative shift that the wider health and social care community is expected to make in future years, as outlined in the White Paper "Our Health, Our Care, Our Say". It is therefore being supported by the Change Agent Team (part of the Care Services Improvement Partnership) in a number of ways. In particular, there is a commitment to extracting the learning from the pilots and disseminating this as effectively as possible to the wider health and social care community.
Promoting Independence Self Assessment Tool:
A tool to enable health and local government systems to establish their successes and challenges in making the shift towards promoting independence, prevention and early intervention.
A new ring-fenced Social Care Reform Grant has been made available over the next three years to redesign local systems. A core element of the funding is to support all councils to create a strategic shift in resources and culture from intervention at the point of crisis towards prevention, early intervention and enablement.
