Making a strategic shift to prevention and early intervention - New Guidance

New practical guidance has been published to assist local authorities and health communities in making a strategic shift to prevention and early intervention. It draws on the experiences and evidence emerging from the first two years of the Partnerships for Older People Projects (POPP) programme and other related initiatives. The material is aimed at key groups of people:-

  • Those people leading the transformation of social care and implementation of Putting People First
  • Commissioners in health and social care with a responsibility for older people's services
  • Leaders from a range of public sector organisations who are involved in the whole systems work, under the auspices of Local Strategic Partnerships and the Local Area Agreement, to promoted the quality of life of older people.
  • Older People's Champions and those supporting the involvement of older people

Pilot Projects

The POPP programme comprises 29 pilot sites. The first round of 19 sites started in May 2006 and the second round of a further 10 sites commenced in May 2007. The sites are very varried in their approaches. Brief summaries of what they are delivering can be found by following the links below.

Change Agent Team Support

The POPP programme is seen as a very important test bed for the preventative shift that the wider health and social care community is expected to make in future years, as outlined in the White Paper "Our Health, Our Care, Our Say". It is therefore being supported by the Change Agent Team (part of the Care Services Improvement Partnership) in a number of ways. In particular, there is a commitment to extracting the learning from the pilots and disseminating this as effectively as possible to the wider health and social care community.

Self-Assessment Tool

Promoting Independence Self Assessment Tool:

A tool to enable health and local government systems to establish their successes and challenges in making the shift towards promoting independence, prevention and early intervention.
