CAT - The Health and Social Care Change Agent Team

Who where they and what did they do?

The Health and Social Care Change Agent Team (CAT) was set up in 2002 to assist health and social care communities tackle the problem of patients remaining in hospitals longer than necessary (delayed transfers of care).

Since then thier remit has grown, and we now provide advice and support on a wide range of issues that affect the care of older people inlcuding@

planning and commissioning care;
assessing an individual's need for health and social care services
developing better services,
improving mental health, stroke and falls services
Offer targeted help to health and social care communities in reducing delayed transfers of care
Support implementation of the key aspects of the National Service Framework for Older People that impact on delayed transfers of care
Assist with development of a more integrated approach to commissioning and provision of services, particularly in relation to intermediate care and housing. This includes exploring opportunities for developing care trusts and the use of Health Act Flexibilities.
